Membership Applications - Newsletter Subscriptions - Patron Donations

If you are interested in becoming a member of the American Czech- Slovak Cultural Club, simply fill out a membership application form and submit to our club president at the address shown below.  Annual dues are $40.00, which includes the club newsletter.  You can obtain a application form, by mail or electronically..or if you wish, stop by the club and pick one up.    Once your application is accepted you will begin receiving the ACSCC Newsletter.

Obtain a Membership Application by Mail
Simply request a membership application form by writing or calling or emailing the President of the American Czech-Slovak Cultural Club, Robert J. Petrik. The address, telephone number and email address are shown below.

The application form available from this page is a file in Adobe Acrobat format (pdf file). You need to have an Adobe Acrobat Reader installed on your computer to make use of this file. Current versions of the Netscape and Internet Explorer browsers usually include an Acrobat Reader,therefore you may already have it.

Obtain a Membership Application Form at the Club
If you would rather stop by and pick up an application we would be glad to see you. Check our Calendar of Events for the club hours. Sunday afternoon is usually the best time. Just ask the person at the door, as you enter, for a membership form, and while your there you may wish to check the menu and stay for a Czechoslovak dinner at our restaurant.

Fill Out the Application Form
After you have your membership form filled it out and send it to us at the address below.  Do not send any money at this time.  Annual membership dues are $40.00, however this can be paid after your application is accepted.

Upon receipt, your completed application will be reviewed and you will contacted.   We hope to see you soon at our clubhouse for Sunday dinner and any of the many club events.

Newsletter Subscriptions 
ACSCC members automatically receive our newsletter by mail.  Non-members may subscribe to the ACSCC Newsletter for $7.50 per year.  If you wish to purchase a newsletter subscription please send us a note stating your request, including your name and address plus a check made out to the American Czech-Slovak Cultural Club for $7.50 to the address below.
